Dave Van Dyke presents: “San Francisco Bay Bridge Eyebar Failure October 2009 Overview”
Over Labor Day 2009 cracking was found on one of the eyebars on the Oakland to San Francisco Bay Bridge. A repair was implemented over the weekend, which subsequently failed and fell onto the bridge deck in October 2009 during an evening commute. VGO worked on the subsequent final repair in December of 2009.

Ken Silva presents: “TEPCO Fukushima 1 Failure”
A first order overview of process and facility design constraints impacting the Unit 1 reactor meltdown. This short summary will review the role of active backup, standby and emergency core cooling systems, the placement of the motor control center and the impact of these two on the ability of the operators to manage the shutdown of the Unit 1 reactor following the March 2011 earthquake and tsunami. Ken Silva was previously Manager of International Product Service for GE Power Systems and was responsible for evaluating operating problems and failures at GE’s international power stations. This presentation will use information available from public sources.
